Darkest Dungeon II is the sequel to Darkest Dungeon. It launched on the Epic Games Store in early access on October 26, 2020. The game was revealed on February 19, 2019 with a teaser titled "The Howling End". With Wayne June reprising his role as the Narrator, the RPG combat gameplay of Darkest Dungeon will return in a "tuned-up" form, and the metagame will feature a "completely different" structure involving a "grueling journey" instead of dungeon-delving in a single locale.

Darkest Dungeon II is a turn-based RPG where players take on the role of a group of heroes, setting off on a journey to a faraway, foreboding mountain to defeat the source of a great evil that is threatening the world. The player's party consists of four heroes, and the player can choose which ones to take into battle, with the option to hire new or replacement heroes at the Inn. The player's party members can interact with each other, affecting their relationships. Heroes' abilities are dependent on skills instead of armor, and skills can be upgraded at the Inn. Heroes can equip provisions to use them in battle, and trinkets are not persistent, meaning any trinkets obtained are lost upon party death or playthrough victory. Darkest Dungeon II is a turn-based RPG turn-based RPG that features a "tuned-up" form of gameplay from its predecessor. The metagame of Darkest Dungeon II will have a "completely different" structure involving a "grueling journey" instead of dungeon-delving in a single locale.

The graphics of Darkest Dungeon II are not yet released, but should be comparable to the graphics of the first game. The graphics in Darkest Dungeon II are similar to the first Darkest Dungeon, but with more detail. There are new monsters and more detailed heroes. There is also new content that was not in the first Darkest Dungeon.

The game features an "infinite" number of regions that can be played in any order, with random modifiers, layouts, and goals. This means that no trip through a region is exactly the same. For example, during one passage of The Sprawl, the party may be granted extra rewards, while during another, they may be required to avoid a specific encounter, such as a mini-boss, or suffer severe consequences. The game includes both procedurally-generated dungeons and hand-designed areas.

Darkest Dungeon 2 provides a variety of routes and goals. It is possible to avoid combat situations, but this can leave the party unprepared for the final boss. Players can choose the region that they will go to next, but the routes are randomized. This means that there is no guarantee that two players will get the same map.
